What is collected
Two forms on this site collect information, and nothing else does.
The contact form collects your first name, last name, email address, organization if you provide one, the inquiry type you select, and your message.
The newsletter form collects your email address, and your first name if you provide one.
Both forms also record the page you submitted from, the page that referred you, your browser's user agent string, and the time of submission.
Your IP address is never stored. A one-way SHA-256 hash of it is stored instead, combined with a secret held on the server. The hash supports rate limiting so the forms cannot be abused. The original address cannot be recovered from it.
Why it is collected
Contact form submissions are collected so I can reply to you. Newsletter submissions are collected so I can send you the Model Optimal Care Brief.
Where it is stored, and who else sees it
Submissions are stored in a Supabase database that only I can access. Notification emails are sent through Amazon Simple Email Service.
Newsletter signups are also sent to FluentCRM, which manages the mailing list. FluentCRM is self-hosted on modeloptimalcare.com, a site I own and control. It is not a third-party marketing processor and your address is not passed to one.
A contact form submission is sent to FluentCRM only if you tick the optional consent box. If you leave it unticked, your message reaches me and goes no further.
Nothing is sold, rented, or shared for advertising.
Consent
Where you give consent, the site records which version of the notice you were shown and when, so that record reflects the exact wording in front of you at the time. The current newsletter notice reads:
Submitting this form subscribes you to The MOC Brief, a short email on healthcare cost containment from Model Optimal Care, sent twice a week on Tuesdays and Fridays. You can unsubscribe from any email.
How to unsubscribe
Every email carries an unsubscribe link, and using it removes you immediately. You can also email me and I will remove you by hand.
Analytics and cookies
This site uses Google Analytics 4 to count visits. It does not load until you accept it. The consent banner defaults to declining, and declining leaves no analytics cookies. No advertising or cross-site tracking cookies are used.
Retention and deletion
Contact submissions are kept while they are useful and reviewed periodically. Newsletter subscriptions are kept until you unsubscribe.
To have your data deleted, email me and say so. I will remove your submissions and your list membership and confirm when it is done.
